FDLE Approved Live Scan Vendor
Fingerprint Card Scan Service must be used by applicants who require a level 2 background check and
live outside of the State of Florida, or are close to our live scan fingerprint location in Florida
and need to obtain a Florida license for certain professions, such as nursing and medical doctors.
(Note: we cannot process card scans for AHCA.)
Florida regulation mandates out-of-state applicants must submit electronic fingerprints with the
appropriate State of Florida ORI numbers provided by the State agency requesting the background
check. Applicants can utilize standard FBI fingerprint cards (FD-258) or FBI FD-258 form.
Note: All data fields highlighted in yellow
must be written/printed legibly and completely filled-in on the fingerprint
card or form to expedite the fingerprint card scan service.

Simply mail from anywhere in Florida, anywhere in the U.S., or around the World a completed FBI
fingerprint card (FD-258), or FBI FD-258 form,
your payment in the form of a money order or cashier’s check made payable to Accurate Biometrics, or
a completed credit card form,

The fingerprint cards are converted to Live Scan the same
day they are received.
Note: Department of Health (e.g. Board of Nursing, Board of Medicine) applicants by-mail can get a
background check for licensing or endorsement
purposes ONLY. To work in the state of Florida, a photo must be taken as part of the background
check process. This allows the applicant to show up in the Clearinghouse a database where background
checks can be shared solely for Florida state agencies. This process cannot be done by mail, and it
is described on the Department of Health's website at: http://www.flhealthsource.gov/out-of-state-providers
under hard scanning section. It is important to check the Department of Health's requirements for
your background check process.
